To elaborate, you don't need to worry about hipaa compliance just like in Google Forms. You can collect patient health information in a secure and safe fashion through JotForm. JotForm puts more advanced features on the table compared to Google Forms. If you're going to arrange appointments and collecting consent forms, JotForm is much more suitable tool for you as opposed to Google Forms. But I think your goal is a little bit more than that. You can create slots in Google Calendar by clicking on the date you want to set appointment slots for and then, click on 'Appointment Slots' button.
